LTO Registered Plates and ORCR

FOR REGISTERED OWNERS

• Photocopy of one (1) valid ID with three (3) specimen signatures.

AUTHORIZED REPRESENTATIVE (IF OWNER IS NOT AVAILABLE)

• Authorization letter
• Photocopy of one (1) valid ID of the registered owner and representative with three (3) specimen signatures.

FOR CORPORATE

• Authorization letter from authorized in-charge.
• Photocopy of one (1) valid ID and company ID with three (3) specimen signatures.
• Secretary’s certificate or board resolution where signatory in the authorization letter is an authorized in-charge.
• Photocopy of one (1) valid ID of the representative with three (3) specimen signatures.

FOR GOVERNMENT

• Authorization letter from Municipal or City Mayor.
• One (1) valid ID and Mayor’s ID with three (3) specimen signatures.
• One (1) valid ID of the representative with three (3) specimen signatures.
